Vials: five, ten, fifteen milligrams. Schedule: 2500 micrograms weekly; increments of 2500 at four-week intervals; 15000 from week twenty-one. Evidence grade A.

Reference mix: fifteen milligrams in three millilitres; 5000 micrograms per millilitre; opening dose at 0.5 millilitres — beyond a 0.3 barrel, inside a 1.0. Capacity conflicts are flagged on every recalculation, with dilution or a larger barrel as the stated remedy.
